Orange County Health Services-Corrections is seeking highly qualified individual to fill a Senior Community Services Worker position.
This position would work with clients at the Orange County Correctional Facility to determine client eligibility for the Ryan White program in order to access HIV treatment. This position would link eligible clients to HIV services and maintain progress documentation in the electronic management system. This position is funded by the U.S. Department of Health and Human Services Ryan White Grant. Work is performed under the supervision of the Corrections Health Services Program Administrator, and is reviewed through file reviews and service verification's, conferences, records, reports, observation, and results achieved.
Serves as liaison and coordinates discharge planning within and between divisions, courts, government and community agencies.
Makes appropriate referrals and advises clients of available program and community resources.
Attends and conducts regular cross-functional team meetings; confers with superiors on difficult assignments; prepares reports and correspondence on assigned cases, maintains related status reports and statistical information.
Assures Program compliance with County, Division, contract and accreditation requirements.
Other duties as assigned.
Bachelor’s degree in Social Work, Sociology, Counseling, Psychology, Health or Behavioral Sciences or a closely related field and three years of directly related social service work experience or a Master’s degree in Social Work (MSW) and one year of directly related social service work experience.
May be required to possess and maintain a valid Florida Driver’s license and provide own vehicle for job related duties.
Must be able to demonstrate intermediate to advanced proficiency in word processing functions, preferably Microsoft Word and email functions.
Experience working in a Correctional setting
Experience working with at risk HIV clients
Case management experience
Bilingual (Spanish or Creole and English)
